FAQ What is a balance bill?Balance billing occurs when a provider bills a member for the difference between what the plan allows for a medical service versus what the provider chooses to charge. Deductibles, copays and coinsurance are not examples of balance billing and you are still responsible for these cost-sharing items.
I've received a bill from my provider, what should I do?If you receive a bill, compare it to the Explanation of Benefits you received. If you are being asked to pay more than what is shown as patient responsibility, you are being balance billed and need to report it.
How do I report a balance bill?Call Sisco Member Services (800) 457-4726OR you can submit the case directly to 6 Degrees using this form: New Case Submission Form *Calling Sisco is preferable as they can verify if your bill is in fact a balance bill. They will then submit the case to 6 Degrees. *Once your case is submitted, you will be asked to complete a Release of Information form so that 6 Degrees can communicate with the provider on your behalf.
What if the provider continues to contact me? If you receive additional bills or communication from your provider, you need to send this information/documentation to your contact at 6 Degrees.
